Key Responsibilities:
Assist in planning and executing project transitions, ensuring all deliverables are met on time.
Collaborate with stakeholders to understand project requirements and scope.
Support in documenting transition plans, processes, and procedures.
Monitor project progress and identify potential risks or issues, escalating them as needed.
Facilitate communication between different teams involved in the transition process.
Contribute to post-transition reviews and gather feedback for continuous improvement.
